Notice on the Updated Requirements for the Health Code Applications

(Effective from January 1,2022. Should there be any discrepancy between the previously issued regulations and these requirements, the requirements in this notice shall prevail)

     In order to proceed with the work of pandemic control and to ensure safe and orderly cross-border travels, Chinese Embassy in Indonesia updated the requirements for the applications of Health code or Health Declaration Form(here in after referred to as "Health code") for China-bound passengers in view of current epidemic situation. The updated requirements will come into effect on January 1, 2022 (Saturday) at 0 o'clock:

1.Pre-departure quarantine

     (1)Company employees are required to take a 21-day closed-loop quarantine before their departure and monitor their health on their own. If one develops symptoms such as fever, cough, etc. , the itinerary should be terminated immediately and such requirements also apply to their close contacts. All companies should issue certificates of employment for their employees. The authorized person in charge of the company should sign and stamp the official seal on the certificates of employment (see Annex 1).

     (2)Other China-bound passengers, should take 21 days self closed-loop quarantine and fill-in the self-monitoring for health condition form (see Annex 2).

     (3)Airlines should implement closed-loop quarantine 7 days before boarding. First, conduct a nucleic acid pre-test. The 7-day quarantine of airlines can be included in the 21-day quarantine before boarding.

2.Pre-departure tests and submission materials requirements

     For those who are negative in the airline's nucleic acid pre-test and have completed the airline's 7-day quarantine to prepare for boarding, the pre-departure test shall be carried out according to their own category. The tests shall be carried out by airlines company in assisting on selecting a designated medical institutions for COVID-19 tests, see Annex 3 for the list of designated medical institutions. When accepting the test, the applicant must present their valid identity documents such as passport to cooperate with identity verification during the tests. If applicants picture cannot be clearly identified, the test report will be deemed invalid. The relevant testing and submission materials requirements are as follows. In order to improve the efficiency of the review and strive to pass the review once, please upload them in the order listed, paying attention to the materials related to testing first, and then general materials. In order to ensure the effectiveness of prevention and control, the Embassy China in Indonesia has the right to require applicants to undergo nucleic acid, serum or lung CT/X-ray tests. Please understand and cooperate.

     (1)For applicant without history of COVID-19 infection, and have been vaccinated with inactivated COVID-19 vaccines such as Sinopharm and Sinovac for more than 14 days according to the procedure:

     【Testing Requirements】Two nucleic acid tests shall be conducted within 48 hours before boarding by two different designated medical institutions and using different reagents at an interval of more than 24 hours.

     【Submit Materials】

     1.Vaccination certificate for the whole process. If you are vaccinated in China, please upload China’s "International Travel Health Certificate". If vaccinated in Indonesia, upload the vaccination certificate issued by the Indonesian government.

     2.Applicants who have received vaccines should provide Vaccination Certificate and Letter of Commitment on COVID-19 Vaccination (see Annex 4).

     3.One nucleic acid pre-test report of airlines 7 days before boarding and two nucleic acid test reports within 48 hours.

     4.General Materials (Specific content exclude later).

      (1)Company employees should provide certificates of employment (see Annex 1).

      (2)Applicants other than company employees, must provide self-monitoring for health condition form (see Annex 2), the identity certificate must be issued and signed according to the requirements of Annex 1.

      (3)Personal valid international travel document information page, Indonesian visa or residence permit page, the most recent border inspection stamp of exit from China and entry to Indonesia.

      (4)Other materials required by the embassy.

     (2)For applicant without history of COVID-19 infection, and the COVID-19 vaccine received is a non-inactivated vaccine:

     【Testing Requirements】One nucleic acid test and one serum IgM antibody test for N protein shall be carried out in the designated hospital 48 hours before boarding.

     【Submit Materials】

     1.Vaccination certificate for the whole process. If you are vaccinated in China, please upload the China’s "International Travel Health Certificate". If vaccinated in Indonesia, upload the vaccination certificate issued by the Indonesian government.

     2.Applicants who have received vaccines should provide Vaccination Certificate and Letter of Commitment on COVID-19 Vaccination (see Annex 4).

     3.One nucleic acid pre test report, one nucleic acid test report within 48 hours and one serum IgM antibody test report for N protein 7 days before boarding.

     4.General materials.

     (3)Those who have not been vaccinated against COVID-19:

     【Testing Requirements】One nucleic acid test and one serum IgM antibody test shall be conducted in the designated hospital 48 hours before boarding, and the results must be negative.

     【Submit Materials】

     1.1 copy of nucleic acid pre-test report of the airline 7 days before boarding, 1 copy of nucleic acid test report and 1 copy of serum IgM antibody test report within 48 hours.

     2.General materials.

     (4)For applicants with the history of Covid-19 infection

     Those who have been diagnosed with Covid-19, or who have not been diagnosed but whose nucleic acid test or serum IgM and IgG antibody test results are positive before vaccination, and those who have received non-inactivated vaccines while have positive serum IgM antibodies for N protein applicants are all aplicants with a history of previous infections.

     【Testing Requirements】

     1.Recovery. Waiting for nucleic acid to turn negative: conduct the first and second nucleic acid tests at an interval of at least 24 hours, and obtain 2 negative reports. For lung CT or X-ray diagnosis, the diagnosis result shall indicate that there is no inflammation in the lung, otherwise it is regarded as invalid. It is recommended to select designated hospitals such as Medistra and Siloam, and pregnant women can be excluded from lung CT or X-ray test. This test is related to the time sequence of the first and second nucleic acid tests, and there is no mandatory requirement. After obtaining the above two nucleic acid negative reports and one lung CT / X-ray diagnosis result without inflammation, it is deemed to have completed the recovery process.

     2.After the completion of the recovery process, if you have been in good health for at least 6 weeks from the issuance date of the last nucleic acid test report, please go to the designated hospital for two nucleic acid pre-tests. The detection should be more than 24 hours interval, 2 times by different hospitals or using different reagents, using nasopharyngeal swabs, and third and fourth negative reports.

     3.After completing the above procedures, applicants should conduct self-quarantine and health monitoring for at least 14 days. If there is no other abnormality during the quarantine, applicants can participate in the nucleic acid pre-detection and 7-days closed-loop quarantine. Test at designated hospital within 48 hours before boarding:

     (1)Those who are vaccinated with the inactivated vaccines against COVID-19 such as Sinopharm and Sinovac should undergo 2 nucleic acid tests.  

     (2)Those who have not been vaccinated against the COVID-19 vaccine (including those who have not followed the prescribed procedures and doses) should undergo 1 nucleic acid and 1 serum IgM antibody test.

     (3)For non inactivated vaccine, 1 nucleic acid tests and 1 IgM antibody tests for N protein were carried out.

     【Submit Materials】

     1.Recovery process materials. The 1st and 2nd nucleic acid negative report + 1 lung CT/X-ray report.

     2.If vaccinated, upload the vaccination certificate. If you are vaccinated in China, please upload the China’s "International Travel Health Certificate". If vaccinated in Indonesia, upload the vaccination certificate issued by the Indonesian government.

     3.Applicants who have received vaccines should provide Vaccination Certificate and Letter of Commitment on COVID-19 Vaccination (see Annex 4).

     4.The 3rd and 4th nucleic acid negative reports after 6 weeks of recovery.

     5.1 pre-test nucleic acid negative report before 7-days quarantine by the airline.

     6.Corresponding 2 test reports within 48 hours before boarding.

     7.General materials.

    Special Notice: Applicants with history of COVID-19 infection have more application materials. Please indicate dates and items in the report and strictly control the above requirements.

     (5) For applicants who have contacts with COVID-19 confirmed cases or asymptomatic infected persons

     Applicants have contacts with COVID-19 confirmed cases, suspected cases or asymptomatic infected persons should conduct self-quarantine and health monitoring for at least 14 days before scheduled traveling to China. If he is healthy, he can participate in the airline's nucleic acid pre-test and 7-day closed-loop quarantine, and accept the test 48 hours before boarding and submit an application according to the above requirements. 

     If the nucleic acid test result shows slight virus reaction, or the test result is "uncertain" or "unidentified", the application will be processed according to the requirements of this article.

     (6) Ship Crew

     After landing, 1 nucleic acid tests are required, followed by 21 days of quarantine observation. The uploaded materials should include negative reports  of the first nucleic acid test.

3. Matters needing attention

     (1)Date calculation Rules

     The starting time of pre-departure tests and health monitoring does not include the day of flight. For example, if the flight is on January 15, 7 days before the departure date is January 8. Pre-departure quarantine and health monitoring time shall be calculated according to the above rules.

     (2)Transitional Rules

The notice will be effective from January 1, 2021. Those who will board within 48 hours or who have been in the closed-loop quarantine can apply for health codes according to previous regulations.

     (3)Application deadline 

     To ensure that the health code is issued, the applicants going to China should apply for the health code at the first time after obtaining the test report and preparing the necessary materials, no later than 5 hours before boarding.

     Please submit the materials as required and pass each material only once. If you are required to return the supplementary material, please submit within 2 hours.

     If the application materials are incomplete or the authenticity is doubtful, the applicants themselves shall bear all the responsibilities of the health code.

     (4)For China-bound passengers transiting via Indonesia

     Passengers who start from a third country without direct flights to China and transit to China through Indonesia must first test at the place of origin and apply for a health code from the Chinese Embassy and Consulate at the place of origin. If there is no application record, the Chinese Embassy in Indonesia will not issue a health code for passengers transferred to China.

4.Liability

     All passengers to China should abide by relevant regulations on epidemic prevention and control and truthfully submit tests reports and other relevant materials. Those who intend to conceal their illness, alter tests reports, disobey quarantine regulations or provide fake information shall not be issued with Health code or Health Declaration Form. Those who violate the law shall take legal responsibility accordingly.
